All students who participate in middle school sports are required to complete a sports physical and turn in the required paperwork prior to participating in team tryouts.

Students participating in middle school athletics are required to complete a sports physical annually prior to team tryouts.
Free Sports Physicals for Tempe Students
Sonoran University Medical Center provides free sports physicals for all Tempe Elementary students every Tuesday and Thursday from 3 to 7 p.m., by appointment only.
